This page explains how this website and general email are used. It does not replace the privacy practices that apply to clinical records.
What this website collects
The contact form prepares an email on your device. It does not transmit or store your answers on this website and is not a clinical intake process. Technical services necessary to deliver the site may process technical data, such as IP address and browser data, to operate it.
General email
If you choose to send the prepared email, Centre Summit may receive your name, email address and message content so it can respond to your question. Email is handled by your email provider and Centre Summit's email system. It is not a secure channel for highly sensitive health information.
What to avoid sending
Please do not include urgent medical details, crisis information or highly sensitive treatment information in regular email. This site does not create a clinical intake record online.
Retention and use
General emails are used to respond to your request or help identify a next step. If you become a client, the privacy practices that apply to clinical records will be explained as part of care, separately from this website.
Privacy Officer
Centre Summit's Privacy Officer can respond to privacy questions, access or correction requests, and privacy complaints. Contact the Privacy Officer at info@centresummit.ca.
Immediate and other support
Centre Summit is not an emergency or crisis service. Call 911 for immediate danger, overdose risk, severe withdrawal or urgent medical support.
